Banner has a specific cover page! PLEASE vet all locations to the travelers and add all experienced departments candidates are willing to ACCEPT/work in. This helps us get candidates presented quicker as we all know these orders open/close quick!

This form needs to be 100% accurate and have all request noted in the highlights section.

• Start Date should be on or very quickly after the Order Start Date listed.
• All RTO should be listed on the cover page, must be less than 7 days.
• RTO will not be approved in the first week of assignment or holidays.
• All facilities the traveler is open to working in should be listed on the submission.
• If interested in any location, please include what states (I.E. Any AZ, Any CO, Any state, etc.)
• All shifts the traveler is open to working on should be listed on the submission.
• All departments the traveler is open to working must be checked on the cover page.
• All candidates submitted should be preclosed and ready to accept.
• Please let us know if the traveler withdraws or is no longer available.

Requirement description :

• Provider must have 24 months minimum of current Emergency Room experience - [REQUIRED]
• Certifications must be American Heart Association or American Red Cross - [REQUIRED]
• Nursys cannot show any hits on license at time of submission - [REQUIRED] [DOCUMENT]
• Banner will not approve RTO within the first week or holidays. Can not exceed 7 days - [REQUIRED]
• Flu vaccination - Banner declinations can be applied for (reach out for forms) **Please add vaccination status in the submission notes**

• Active American Heart Association BLS - [REQUIRED] [DOCUMENT]
• Active American Heart Association PALS - [REQUIRED] [DOCUMENT]
• Active TNCC Certification (Trauma Nursing Core Course) - [REQUIRED] [DOCUMENT]
• Flu vaccination - [REQUIRED]
• Driver's License - [REQUIRED]

CERTIFICATION REQUIREMENTS :

• ACLS (AHA)
• PALS (AHA)
• TNCC
• BLS (AHA)

STATE LICENSE REQUIREMENTS :

• Wyoming

ADDITIONAL LICENSE REQUIREMENTS : Must have active license at time of submission
Weekend Requirements : Weekend schedule to be determined by facility
On Call Requirements : On call schedule to be determined by facility
